Mental Health Assessment
HOME / Mental Health Assessment
At HPC, we understand the importance of timely and accurate mental health support. If you are unsure about what your symptoms mean, we can help. We provide diagnostic assessments tailored to unique individual experiences. Our mission is to empower you to move forward with confidence in your mental health care plan. Let’s work together toward clarity, growth, and holistic well-being.
Nurse Practitioners are licensed professionals who have the authority to conduct psychiatric assessments, interpret test results, communicate a diagnosis, and prescribe medication for treatment.
Our Nurse Practitioner, Shannon Wright, has expertise in mental health and addictions. She will complete a comprehensive assessment for ADHD and/or other psychiatric/mental health conditions. This assessment will include reviewing a detailed individual history, conducting clinical interviews, reviewing screening tools, and additional diagnostic testing as needed. She will then review these findings with you and if appropriate, communicate a diagnosis. Shannon will provide treatment recommendations and together you will develop a treatment plan that may include medication, psychological interventions, and/or psychotherapy.

“The comprehensive mental health assessment is intended to gather information to help inform your mental health evaluation. However, the completion of the assessment does not guarantee a formal diagnosis. The results of assessment are a part of the overall diagnostic process and should not be construed as a definitive diagnosis on their own.
We cannot see patients from other provinces at this time.”

Frequently Asked Questions
The intake assessment forms will ask you to share your personal and family history, current and past symptoms, details of previously received diagnoses, and information from nominated family members on the forms provided. If you are referred to the Nurse Practitioner (NP) by a psychotherapist on our team, then the NP will include the referral information in the intake. You may also be asked to complete routine blood work.
Following the completion of intake forms, there will be a comprehensive mental health assessment appointment with the NP that will discuss and record details of your symptoms, and functioning. This comprehensive mental health assessment is typically completed and finalized in one session; however, if additional time is needed, you may have a follow-up appointment to continue the assessment at no extra cost.
Based on your comprehensive mental health assessment, the NP will review the results during your assessment appointment. At this time, If relevant, the NP will also communicate a diagnosis and provide treatment recommendations. The NP will review this with you in detail, and you can discuss your questions and concerns to identify the plan that meets your unique needs.
If you consent, we will share the consultation report with your primary care provider or your child’s pediatrician.
Depending upon your treatment plan you may be prescribed medicine and/or focused psychotherapy. If your treatment plan requires medication, then the NP will prescribe and do periodic follow-ups to assess and optimize this treatment.
No, this service is open to everyone from December 2024. We are an ethically run practice and want to offer a speedy and affordable process of evaluation to our clients.
The comprehensive mental health assessment is intended to gather information to help inform your mental health evaluation. However, the completion of the assessment does not guarantee a formal diagnosis. The results of assessment are a part of the overall diagnostic process and should not be construed as a definitive diagnosis on their own.
If your comprehensive mental health assessment does not reveal a formal mental health diagnosis (i.e., ADHD, PTSD), this does not mean that your symptoms and concerns are irrelevant. It is important for you to understand that while we prioritize providing you with an accurate professional evaluation, we do not dismiss the challenges you may be experiencing. Therefore, our NP will review alternative explanations for your experiences and presenting symptoms. We will continue to work closely with you to identify an individualized treatment plan that supports your needs, including referrals to counselling or other specialists. We are committed to your well-being and will support you in finding the best course of action.
Making an ADHD diagnosis is complex. There are individual differences in symptom presentation, behaviors, and functional abilities. Therefore, an ADHD diagnosis requires a thorough formal assessment from a clinician who is authorized to assess, diagnose, and treat.
ADHD treatment is multifaceted. Your mental health needs are informed by your current and past symptoms, experiences, and medical and family history. Thus, your treatment plan will be individualized, and evidence-based and may include a combination of medication and/or therapy.
Parents are generally involved in the information intake phase and comprehensive mental health assessment. Parents are also included in the discussion of diagnosis and treatment recommendations. Parents are encouraged to ask questions during this discussion and are encouraged to co-evaluate the risks and benefits of each treatment recommendation as the treatment plan is created. We seek consent from parents and clients every step of the way. In compliance with Ontario’s regulated healthcare practice, we are fully committed to maintaining confidentiality at all times.
If you have a confirmed diagnosis, you may consult with us to review or adjust your treatment plan.
Yes, we can provide supporting documentation, or recommendation if that supports your educational or occupational functioning. The service of an NP is a non-insured health benefit, similar to massage therapists, or physiotherapists. As such it is not covered under OHIP. Patients may pay out of pocket or hold additional health insurance benefit that covers Nurse Practitioner services. You may contact your insurance company to check if the benefit is covered in your current plan.
Yes, if you have a confirmed psychiatric diagnosis and would like to review and adjust your treatment plan only for medication. You will be required to submit a recent psychological or psychiatric report confirming your diagnosis to book this type of appointment. The NP will review the report before meeting with you. If you have more than one mental health diagnosis and the NP requires a more thorough assessment to confirm the diagnosis, you may require a comprehensive mental health assessment. The fee for this visit will vary depending on the level of complexity.
